Starting Pay : $55,068 Base Salary with potential increases depending on education and experience.

Position Purpose : This position is responsible for providing outreach and engagement phone calls and letters to UCare Connect and UCare Connect plus Medicare members to offer Care Coordinator services.
Daily responsibilities include telephonic outreach, written correspondence, documenting and tracking outreach and providing support to the SNBC UCare Connect and UCare Connect Plus Medicare team.
The position is responsible for providing telephonic Health Risk Assessments as requested and Transitions of Care as assigned.
Other duties may include processing paperwork, faxing, data entry, internal and external customer service, maintaining case files, updating information, interviewing clients and additional opportunities as assigned.

Main Duties and Responsibilities :
- Manage a roster of UCare Connect and Connect members who are currently inactive and ensure outreach is completed within established timelines.
- Monitor activity on assigned roster and respond to notifications of activity, including but not limited to transitions of care, requests for care coordination and provide annual outreach to engage the member in care coordination.
- Provide multiple attempts in multiple forms as required to members 60 days prior to their annual Health Risk Assessment / Refusal date.
- Document all outreach attempts, engagement, and transitions of care updates in the Electronic Medical Record.
- Communicate with collateral contacts to assist in reaching members during times of transitions of care and annual reassessment outreach.
- Provide a person-centered, welcoming approach to encourage members to engage in care coordination services.
- Work collaboratively with Team Leader to monitor timelines for engagement and Health Risk Assessments / Refusals.
- Additional responsibilities include, but are not limited to faxing, answering phones, emailing, and using a copier and postage meter.
Minimum Education and Experience :
- Four-year degree a behavioral science or related field and at least 2,000 hours of experience providing services to individuals with a mental illness or substance use disorder.
- Preferred Minnesota social work license in good standing.
- At least two years of experience in administrative / office support or phone outreach position.
