JOB SUMMARY

The RN conducts assessments and provides ongoing monitoring of clients’ psychiatric history and physical health. Dispenses medication, advises on medication effectiveness and side effects, and educates clients about medication management. Provides service coordination, supportive counseling and symptom management for both clients and their family members. The RN’s wellness coaching responsibilities include nutrition counseling, physical health screening, promotion and developing individual and group activities focused on overall health and wellness.

  • Provide service coordination for a shared group of clients.
  • Provide individual supportive counseling and symptom management. 
  • Conduct comprehensive assessment of psychiatric history and physical health needs; develop and implement treatment goals in conjunction with the treatment team.
  • Provide on-call, after-hours, and weekend/holiday services as required by contract. 
  • In collaboration with the treatment team, participate in the organization and management of medication coaching and delivery to clients, as well as integrating medication administration into treatment plans. 
  • Dispense and account for all prescribed medication. Responds appropriately to medical emergencies. 
  • Assist in administering Patient Assistance Program, ensuring efficiency and accuracy in all aspects. 
  • Complete all charting and documentation as contractually required and in a timely manner as dictated by program requirements. 
  • Ensure that all medical policy and procedures are followed and meet regularly with Program Leadership to monitor this. 
  • Assist Psychiatrist in administering and evaluating medications prescribed by Psychiatrist, evaluating medication effectiveness, side effects, and complications; and arrange for lab work according to protocol. 
  • Participate in and represents program at Medication Monitoring Committee Meetings; act as liaison with Program Psychiatrist to obtain corrections to documentation and adjustments to procedures as needed. 
  • Introduce clients to Common Ground software, assisting clients in completing the Common Ground Health Report before medical office visits. 
  • Transport clients in personal and/or company vehicles as required.
  • Support the team in providing case management and rehabilitation visits when needed and when directed by Program Leadership. 
  • Serve in a mentoring role to staff and interns regarding pharmacology and patient care. 
  • Comply with all MHS and program rules, policies and procedures, including HIPAA standards.

QUALIFICATIONS

  • Possession of a 4-year BS degree in nursing from an accredited college or university
  • Minimum of three years of experience treating psychiatric and dually diagnosed populations
  • Demonstrated knowledge of Assertive Community Treatment model as it relates to patient care
  • Bilingual in Spanish preferred
  • Ability to work both independently and to effectively collaborate with a team
  • Ability to work with minimal direction and supervision
  • Ability to interact positively with culturally diverse populations and maintain the professional distinction between therapeutic and social relationships
  • Ability to react appropriately to emergency situations
  • Proficient user of Microsoft Office Suite and EHR software
  • Able to effectively communicate verbally and in writing
  • Valid California driver’s license and current auto insurance coverage
