JOB SUMMARY
The LVN (Wellness Coach) provides case management for clients including coordinating and monitoring the activities of the treatment plan. The incumbent will complete and oversee assignments to improve the diet, nutrition, and wellness of the clients.
RESPONSIBILITIES

• Provides service coordination for an assigned group of clients. Assumes responsibility for developing, writing, implementing, and evaluating overall treatment goals in conjunction with the treatment team.
• Provides individual supportive counseling and symptom management. Educates and supports clients and family members.
• Introduces clients to Common Ground software, assisting clients in completing the Common Ground Health Report before medical office visits, and conducts evaluation of psychiatric history and physical health needs as part of the treatment team.
• In collaboration with the treatment team, participates in the organization and management of medication coaching and delivery to clients, as well as integrating medication administration into treatment plans.
• Assists RN and MD in medication oversight and accounts for all prescribed medication. Responds appropriately to medical
emergencies. Ensures physician orders are carried out and coordinates medical care.
• Develops curriculum and prepares manuals, visual aids, course outlines and other materials used in promoting health.
• Provides individual and group wellness coaching and education.
• Coordinates with primary physicians and health care personnel to determine physical capabilities and nutritional needs.
• Conducts health and wellness screenings/assessments for assigned clients.
• Assists in administering Patient Assistance Program, ensuring efficiency and accuracy in all aspects.
• Provides on-call crisis intervention covering nighttime hours and works late shift and/or weekends/holidays as scheduled.
• Transports clients in MHS or personal vehicle as required.
• Completes all charting and documentation as contractually required and in a timely manner as dictated by program
requirements.
• Assist Psychiatrist in implementation of medication algorithms such as San DMAP if required by funding source.
• Maintain the distinction between therapeutic and social relationships.
• Ensures that all medical policy and procedures are followed and meets regularly with program leadership to monitor this.
• Assists Psychiatrist in administering and evaluating prescribed medications, evaluating medication effectiveness, side effects, and complications. Arranges for lab work according to protocol.
• Resourcefulness, initiative and the ability to work with minimal direction and supervision.
• Utilizes logical problem-solving techniques both independently and in collaboration with workers in dealing with client needs; prioritize and manage responsibilities as evidenced by timely completion of tasks.
• Demonstrates commitment to professional growth by attending in-service trainings and staff development programs. Seeks out supervision when needed.
• Participates in regularly scheduled staff meetings and Daily Organizational Meetings.
• Comply with all MHS and program rules, policies and procedures including HIPAA standards.
QUALIFICATIONS
• Certification or diploma in vocational nursing.
• Current licensure as a Vocational Nurse in the State of California.
• At least three years’ experience treating psychiatric and dually diagnosed populations.
• Proficient user of Microsoft Office Suite and EHR software.
• Ability to problem solve and react appropriately to emergency situations.
• Ability to work independently and effectively collaborate with a team
PAY RANGE: $30 - $32 / HOURLY
