Earn $24.21/hour
Position Details
The Care Navigator coordinates aftercare services for consumers in Cumberland and Perry Counties. They ensure timely follow-up appointments within 7 days of inpatient hospital discharge, address social determinants of health, and support individuals to remain in the community for 30 days. The Care Navigator collaborates with primary pare providers, case managers, and other services, making necessary referrals for continued care. Additionally, they participate in Care Coordination meetings and assist with 340b Harm Reduction event tracking.
Schedule: Monday-Friday, 8 am-4 pm
Duties and Responsibilities:
- Collaborate with inpatient hospitals for discharge planning.
- Engage patients, assist with discharge plans, and provide transportation in personal vehicle.
- Ensure all transportation resources for discharged individuals.
- Coordinate referrals and intake processes.
- Participate in Care Coordination Meetings.
- Assist with paperwork and documentation.
- Coordinate follow-up healthcare services.
- Establish relationships with local healthcare providers.
- Contribute to Harm Reduction events.
- Attend trainings and meetings as assigned.

Our ideal candidate will possess the following qualifications:
- Bachelor's degree in human services or social science
- Experience working in a case management role
- 2 years' experience working in the mental health field
- Active driver's license and access to an insured vehicle
