Merakey is seeking aFull-Time Dual Diagnosis (Mental Health and Developmental Disability) Case Managerbased out ofWilkes Barre, Pennsylvania serving counties acrossNortheastern Pennsylvania (Lackawanna, Luzerne, Susquehanna & Wyoming Counties).The Dual Diagnosis Treatment Team (DDTT) program offers support to children and adults who have both a serious mental illness and a developmental disability. Participants receive counseling, physical healthcare, support finding housing, assistance during crises, and help dealing with other issues in their daily lives.
The Case Manager will be responsible for:
- collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
- performing initial intakes, assessments, and timeline summaries.
- providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
- providing individual and group treatment and support according to each individual's/family's needs.
- supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.

- Bachelor's degree in social work, psychology, or another human services related field
- Three years' experience in a mental health and/or developmental disability setting
Preferred:
- at least one year of experience in a mental health setting and one year working in a developmental disability setting
