We are seeking a Drug and Alcohol Case Manager to join our team in our Philadelphia, PA location.
Earn $21.61/hour.
Position Details
Assist with linkage to external resources such as, but not limited to: recovery-based supports such as 12-step meetings, medical benefits, PCP, specialty doctors, food banks, transportation, obtaining and maintaining basic living needs and skills related to housing, food, psychiatric and medical care, transportation, socialization, education, and employment. Also, assist consumers in obtaining valid, state identification when applicable. Coordinate service delivery with other service providers and monitor the client's progress
