Community Health Liaison (Community Nutrition Outreach)

Program Development and Evaluation
- Stands up nutrition and healthy cooking programs designed to meet the needs of the East End community, as well as other historically underserved populations.
- Builds relationships with partners, especially in the East End community, to identify opportunities for new community nutrition programs.
- Coordinates and collaborates with the BSR Community Health team and community partners to identify program gaps and opportunities for improving programs and services
- Prepares and documents program and class outlines, objectives, participation records, evaluation measures, and analysis of behavior changes. Uses data collected to validate effectiveness of services.

Site and Logistics Coordination and Support for CNO Programs
- Identify and secure appropriate sites for delivery of nutrition programs.
- Ensure that programs are designed and delivered to maximize participation and benefits for participants
- Build effective communication tools and plans for ensuring participants remain engaged and informed

Participant Recruitment for CNO Programs
- Leads the participant recruitment efforts for community nutrition outreach programs.
- Liaisons and builds relationships with community partners to establish patient referral pipelines for community nutrition outreach programs.

Lead Community Engagement around CNO Programs
- Builds relationships with community partners, organizations, and businesses that serve and are trusted within the East End community.
- Seeks opportunities for Bon Secours, to promote its programs and services at community events focused on the diverse communities it serves.
- Develop a core group of grassroots advisors willing to offer feedback and ideas on how to improve our East End community health programs and services to best serve the community.
Shared Community Health Team Responsibilities
- Collaborates with the Community Health team in planning and executing community events.
Employment Qualifications
Education -
Required Minimum Education: Bachelors of Science
Specialty/Major: Nutrition from accredited university
Licensing/ Certification -
Licensure/Certification Required: Registered Dietitian in State of Practice
Licensure/Certification Preferred: Certified Diabetes Educator
Minimum Years and Type of Experience: 2 years of experience in chronic disease nutritional counseling
Other Knowledge, Skills and Abilities Required: Knowledge and proficiency in current, evidence-based methods and practices of care delivery, with an emphasis on health promotion and risk reduction. Strong interpersonal skills and ability to work collaborative with patients, non-clinical staff, clinical staff and project team. Effective oral and written communication skills.
