Position Purpose:
The Case Manager collaborates with physicians, social services, nurses, and community agencies to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition for patients and through ensuring that patients are at a medically appropriate level of care.
What you will Do:
- Coordinates/facilitates patient care progression throughout the continuum while advocating for the patients best possible outcome.(Problem Solving/ Communication)
- Works collaboratively and maintains active communication through interdisciplinary rounds with physicians, nursing and other members of the multidisciplinary team to effect timely, appropriate patient management.(Communication)
- Ensures warm handoff with caregivers at next site of care including but not limited to health coaches, home care agencies, and nursing homes or skilled facilities. (Communication)
- Addresses and resolves system problems impeding diagnostic or treatment progress. Proactively identifies and resolves delays and obstacles to discharge.(Problem Solving)
- Conduct face-to- face assessments and document primary and back-up discharge plan on all patients. (Problem Identification)
- Determine appropriateness of hospital level of care including designation as observation or inpatient Level of Care (Problem Identification)
- Conduct case presentation for all admission requiring clarification of patient’s Level of Care or readmission evaluation. (Problem Solving)
- Assists in creating processes to anticipate and address specific situations within the department, including post-acute resource referral lists, denials, readmissions, and Length of Stay. (Initiative)

Minimum Qualifications:
- Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment.
- Case Management Certification preferred.
- Five years Clinical RN experience required
- Diverse knowledge of community services, resources and programs
- Previous case management experience preferred.
