Overview:

Our Award Winning Client, a nationally recognized organization, is seeking a Medical Social Worker to join their dynamic team. In this role, you will provide vital medical social services to patients within their residence, aligning closely with the Plan of Care. Collaborating with physicians and team members, you will address significant social and emotional factors impacting patients' health issues.

Key Responsibilities:

• Conduct comprehensive assessments of patients and families to evaluate their capacity to cope with daily living challenges.
• Participate in the development of the Plan of Care, offering suggestions for enhancements to physicians.
• Deliver supportive casework to aid in restoring patients to optimal social health adjustment.
• Assist families in accessing appropriate community resources or adapting to disability.
• Facilitate discharge planning and engage in in-service programs.
• Serve as a consultant within the healthcare delivery team.
• Aid clients and families in comprehending and adhering to medical recommendations.
• Record and report social and emotional changes.
• Prepare periodic reports for physicians and third-party payers regarding care provided.
• Maintain liaison contact and follow-up with community agencies.
• Coordinate services with existing agencies or facilities involved with clients and families.
• Refer clients to other community agencies as needed.
• Participate in staff and agency development activities.
• Stay updated on professional competencies through in-service attendance.
• Attend in-service sessions, case conferences, and staff meetings as required.
• Assist in developing and revising the physician's Plan of Care.
• Submit assessments and progress notes in a timely manner.
• Supervise Social Worker Assistant according to agency policy.
• Maintain adequate clinical and progress notes and submit all required paperwork promptly.

Schedule and Shift Details:

The work schedule and shift expectations will be based on the assigned caseload. Employees will be required to drive daily for meetings and/or visits to residential homes.

Qualifications:

Required:

• Master's degree or higher in Social Work.
• 2 years of Social Work experience.
• 1 years of post-graduate Social Work experience in a Health Care Setting.
• Valid Regular Driver's License (DL).
• ANY of the following valid licenses/certifications:
• Licensed Master Social Worker (LMSW)
• Licensed Clinical Social Worker (LCSW)

Looking for full-time employment (40 hours per week) but open to part-time (approximately 32 hours per week). Coverage Area: Arlington Heights & surrounding (required to cover entire territory, not extending further north than Crystal Lake Route 167, and not east of I-294). Applicants must possess a Master's degree, IL LSW/LCSW, and a minimum of 2 years of post-graduate Social Work experience in a healthcare setting. Experience in home healthcare within the region is preferred by the team.
