What are the responsibilities and job description for the LOC 1 Case Worker position at MHMR Authority of Brazos Valley?
Job Description: Provides crisis prevention and management, monitoring, assessment, and service planning and coordination for up to 200 adults with mental illness in the Brazos Valley. Routine Case Management involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. The Routine Recovery Facilitator will negotiate and facilitate the array of services and supports needed to address the client’s personal goals and desired outcomes as identified through the TRR Assessment and Individual Recovery Plan and provide Rehabilitative skills training as needed. The Routine Recovery Facilitator is responsible for ensuring planned services and support are implemented and appropriately documented as well as advocating and ensuring consistent interactions with other agencies.
Education: Requires Bachelor's degree in behavioral, social science, or humanities field, educational psychology, or early childhood education or intervention.
Experience: Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are necessary.

Pay: $37,598.00 - $45,819.00 per year
