Job Title: Parent Mentor

Reports To: CSP Program Supervisor

Job Status: Full-time or Part-time

Summary:

This position provides home-based individual and community-based group parenting, and behavioral health and recovery skills-building activities aligned with each client's plan. The services and strategies offered assist in healthy, functional parenting support for children by effective management of parental mental health. This position will maintain a full caseload of 18-25 families, effectively timing cases so that this range is maintained. This position will work as part of a larger internal and external team, coordinating services for each client to ensure all goals are addressed. Timely and accurate documentation of each client interaction is required, with impeccable attention to detail, contract compliance and field standard case noting. This position will also offer supervised visits with families, coordinating the logistical needs to ensure that families are well supported during each visit. Participation in individual and department clinical supervision as required. This position always demonstrates respect for internal and external community, keeps commitments and inspires diverse community trust, resulting in the ongoing public ambassadorship of organizational values and goals.


Case Management 60%

  • Conduct home-based parent coaching and ongoing interventions and support that centralize family experiences through a respectful, trauma-informed lens, resulting in client satisfaction and retention.
  • Manage 18-25 active family cases, resulting in timely support and follow up on all client needs and supports related to case plan.
  • Provide community-based and internal referrals and supports to families as needed or requested.
  • Case note all interactions and ancillary support with each client within 24-48 hours of interaction.

Group Management 10%

  • Offer occasional support groups for parents seeking to explore and acquire new skills in parenting and stress management, resulting in enhanced behavioral health
  • Offer insight to group members that promotes individual confidence, reflection and contribution
  • Case notes a group summary after each session, including progress towards group goals, individual observations of member activities and alignment with case plan goals
  • Prepare a curriculum in advance of group session that tailors to specific group interests and goals
  • Regularly conduct informal and formal evaluations on group satisfaction through culturally intelligent evaluation practices and formal evaluation tools
  • Share data from group evaluation with leadership through an organized collection of monthly and quarterly data collection

Documentation, Compliance and Supervision 30%

  • Enters case notes, assessments and all intake and entry forms for each client and group in a timely and accurate manner, contributing to accurate and on-time billable services being completed successfully.
  • Responds to internal requests for information and clarification within 24-48 hours resulting in the efficient continuation of colleagues' work.
  • Attend weekly supervision with supervisor and engage in active participation

Key Skills and


  • Highschool Diploma, Bachelor's degree preferred
  • Bachelor's degree in family social services, social work, behavioral health/psychology or another related degree
  • 2-3 years working with families experiencing high barriers to success and/or history of child welfare systems engagement
  • 2-3 years working with individuals and families working on addressing behavioral health needs and goals
  • Experience in child development
  • Ability to help coach parents with age-appropriate parenting
  • Experience with resources in the community and know what services to refer families to
  • Spoken fluency in Spanish, Somali, Hmong and/or Karen is preferred, but not required.
  • Strong written communication and comfort communicating by email to share updates and questions with key stakeholders, edits work for spelling and grammar and able to vary writing style to meet needs
  • Ability to manage a calendar proficiently, resulting in a detailed and updated account of all work-related activity
  • Successful experience working on a team and sharing collective and individual responsibility for group goals, contributes to a positive team and work culture, addressing workplace conflict transparently and professionally so that larger service goals are prioritized and actively participates in team meetings.

Salary : $40,000 - $43,000
