Public Health Nurse/Registered Nurse/Social Worker - Carlton County

Carlton County Public Health & Human Services is currently seeking applicants for a full-time (37.5 hours per week) Public Health Nurse or Registered Nurse or Social Worker.  Carlton County has one vacancy and this will be filled at the appropriate level depending upon applicants qualifications. 

Job Summary/Position Purpose Under limited supervision from the Social Service Supervisor or Human Services Supervisor. This long-term care/disability unit position will focus on assessment for community services to individuals, families, and the community. This full-time, benefit-eligible position is located at the Carlton County Public Health & Human Services building with work being performed primarily in client’s homes and in the community. Upon completion of a probationary period, a hybrid schedule between office building, community, and remote work may be approved. In addition, this position will include professional public health nurse duties or social worker duties to ensure the proper administration and coordination of established public health programs per state statute, regulations, and guidance. Collaborate closely with local, regional, state, and Tribal health systems, governmental systems, and other agencies to develop interdependent perform professional registered nurse/PHN or social work duties to ensure the proper administration and coordination of established long term care/public health and disability unit programs. Must be skillful at developing and working with multi-disciplinary teams, families, and general public. Exercise independent judgment, exercise strong oral and written communication skills. Some evening and/or weekend work may be required.

• Provides direct skilled nursing service to clients as recommended by physician.
• Uncovers health problems through observation, interviews and analysis of records and initiates appropriate action.
• Teaches the client basic principles of healthful living applying information from related fields such as nutrition, dental hygiene, safety, etc.
• Teaches and supervises family members and auxiliary nursing personnel proper nursing and physical therapy care of the client as directed by physicians.
• Communicates/consults with the physician, nursing team, home health aides, social worker, and other involved caregivers to coordinate health care plans to meet client needs.
• Performs diagnostic tests as authorized and interprets the findings to client and/or families.
• Instructs caregivers to recognize early signs of disease and deviations from normal health patterns.
• Assists client in obtaining medical supplies and establishes appropriate client service fees.
• Maintains case file documentation.
• Provides school health services to assigned schools.
• Provides input in program planning, the development of agency goals and the evaluation of the agency's programs and policies.
• Participates on pre-admission screening teams, community child protection teams, etc.
• Encourages community development by initiating or participating in action to address specific community needs.
• Represents the Public Health Department by addressing various public and private organizations, community groups, and members of the general public concerning policies, programs, responsibilities and activities to inform and elicit community support.
• May be assigned to work in specific public health program areas, such as WIC, MCH, EPS, etc.
• Provide in-service education to personal care attendants, home health aides, and housekeeping personnel.
• May supervise and evaluate personal care attendants, home health aides, and housekeepers.Public Health Nurse: Certification as a Public Health Nurse in Minnesota and current licensure as a registered nurse in Minnesota. 

Registered Nurse: Current licensure as a Registered Nurse in Minnesota. 

Social Worker: A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field OR A bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.

Carlton County Preferred Qualifications
PHN: Must have RN Licensure from the State of MN Board of Nursing and Minnesota Public Health Nursing certificate; Bachelor’s degree in Nursing from a university or college accredited by the National League of Nursing (PHN) SW: Bachelor’s degree in from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field OR A bachelor’s degree from an accredited four year-college or university with a major in any field and Three (3) years of public health and human service experience with two (2) years in the program area. Or an equivalent combination of education and experience sufficient to perform the essential functions of the job. RN: Must have RN Licensure from the State of MN Board of Nursing and minimum associates’ degree in nursing.
