Job Responsibilities:

Using completed tools assessing the participants medical, physical and environmental status and the Support Planners knowledge assist the participant to plan services to meet their needs.

Implement the agreed upon services.

Continuously assess and monitor the services to assure the quality of the services being delivered.

Evaluate the services to assure they meet the needs of the participant.

Comply with State, Federal and Contract guidelines

Qualifications:

One year experience of case management of the elderly and disabled

Bachelors Degree in Human Services

Must live in Maryland

Valid Driver's License

Type 30 Words per Minute

Knowledge of Microsoft Word, Excel other software

Job Type: Full-time

Pay: $48,000.00 - $52,000.00 per year
