Seeking an RN care coordinator for a primary care clinic in Bush Alaska. This position will coordinate care for patients in several remote locations for chronic care appointments, follow-up for recent hospitalization, and advocate for patient needs.
Experience/Education/Qualification
Experience:
Shift: M-F 0800-1700
Vaccinations: Current COVID and flu vaccines required.

Experience:
- Experience in Care Coordination/Case Manager required.
- Alaska is not a compact state, the applicant must have a current and unencumbered RN license in the home state with an Alaska license application.

- Must be culturally sensitive, respectful, and courteous.
- Strong organizational skills, communication skills with care providers and patients, and computer proficiency, and telephone communication are a must.
- ALL TRAVELERS ARE REQUIRED TO SHOW PROOF OF COMPLETED COVID VACCINATIONS PLUS BOOSTER. FLU VACCINES ARE ALSO REQUIRED AND EXEMPTIONS WILL NOT BE ACCEPTED.
