JOB TITLE : Breast Health Nurse Navigator

JOB SUMMARY

With a patient centered culture, the Breast Health Navigator will provide a positive experience to our patients and community by serving as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive are services offered by the Cancer Center. The Navigator will facilitate patient appointments, including those made with labs, diagnostic areas, and physicians.

Experience/Physical Requirements :

Exerting up to 50 lbs. of force occasionally and/or up to 20 lbs. of force frequently and/or up to 10 lbs. forces constantly. Effective interpersonal communication skills in order to effectively collaborate with physicians and instruct patients and their families. Experience in breast cancer/women’s health preferred. Requires problem solving, decision making and critical thinking. Must be self-directed. Has the ability to implement professional and community based education programs. Requires excellent leadership, organizational, written and verbal communication. Computer literate. Clinical competency to assist with breast procedures.

JOB SPECIFIC RESPONSIBILITIES

Communication Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Outreach Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff etc.) Coordination of Care Follows patients throughout the care continuum, including outpatient, inpatient admissions. Provides comprehensive nursing care to patients in radiology requiring treatments such as biopsy. Collaboration Works with cancer registry, tumor board and cancer committee.
