This Opportunity: The primary responsibilities of the ACTT Vocational Professional include assisting and supporting individuals who are served in the development of skills, behaviors, responsibilities, and supports needed to live, learn, work and be integrated into the community with the greatest possible degree of self-determination and independence. What You'll Do: • Facilitate the vocational services to each individual receiving ACTT services who is interested in work. These duties include but are not limited to career exploration, job placement, on-site training, support to maintain employment, benefits counseling and follow-up support for as long as necessary. • Assist in the development of natural support networks. Facilitate and instruct the individual in establishing and maintaining relationships with co-workers, supervisors, family members, significant others and community members. Plan and develop activities that foster socialization and employment within the community to integrate individuals into society. • Provide monitoring and/or long-term follow-up for persons placed in employment and their employers. • Coordinate efforts with other providers of services as the North Carolina Division of Vocational Rehabilitation, LME staff, benefits counselors and family members to support the rehabilitation process. • Provide a variety of activities needed by the individual to develop the skills, behaviors and responsibilities necessary to live, work, learn, play and socialize in the community with the greatest possible degree of independence. • Teach specialized skill development to the individual in such areas as budgeting, cooking, shopping, dressing appropriately, bathing, using the telephone, writing letters, arranging appointments, developing leisure skills and recreational skills, and medication self-administration. • Plan and integrate rehabilitation services in the individuals Person Centered Plan. • Function as a member of a multi-disciplinary team to develop and implement treatment goals. • Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis. • Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources. • Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual’s needs. • Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team. • Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager. Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments. • Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors). • Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk. • Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes. • Developing service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members. • Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements. • Driving and traveling may be required.
