Bridging Care for Families, a division of Multicultural Community Services is a small, local, non-profit Home Health Care Agency that has been committed to serving our community and its members for more than 19 years. Our agency is deficient-free with DPH. We are Medicare/Medicaid certified.
Our focus of care is providing nursing to medically fragile individuals both children and adults. By teaming up with the caregiver in the home and providing clients with 1:1 quality nursing services we will help to keep them in their own homes.
Required:
- At least 1 year of experience in-home care.
- A Mass. nursing license in good standing.
- Knowledge of computers and specifically DeVero is a plus
- Prefer Respiratory experience to include knowledge of trachs and/or vents, G tube care
Orientation specific to this client’s needs will be provided. 24-hour Clinical supervision is provided. We provide PPE for our staff as recommended.
In many cases the hours can be flexible and to accommodate the nurse’s schedules. Those seeking strictly per diem work are also encouraged to apply.

Monday, Tuesday & Thursday 4:00 pm – 9:00 pm
Westfield
Wednesday & Thursday 8:00 am – 5:30 pm and every other Tuesday 8:00 am – 5:30 pm
