Job Description Summary
The RN Patient Navigator reports to the Executive Director of Marion Medical Center. Under general supervision, the RN Patient Navigator functions in a collaborating role with hospitalists, gastroenterologists, all clinic staff, all outpatient staff within the endoscopy and colorectal care service line. This role facilitates care through participation in clinic, individualized goal-directed care utilizing principles and practices of the nursing process and interacts with the interdisciplinary team members to achieve desired results. Job duties will consist of but will not be limited to scheduling appointments for patients for specialty services and testing (cardiac, pulmonary, etc.) under the direction of the unit’s attending physicians, reminding at risk patients of these upcoming appointments, and follow up and record retrieval of these appointments within our system or from outside institutions. The nurse navigator will communicate each step of the care plan to all pertinent medical staff. These steps will include appointment dates, no shows for these appointments if applicable, and results gathered from appointments. The nurse navigator will play a vital role in the overall health care continuum for qualifying patients in the Transformation Endoscopy Care Program.Entity

Additional Job Description
Requirements (Education, Work Experience, Licensure, Registry &/or Certifications)
Bachelor of Nursing (BSN) degree from an accredited college/university preferred; Master of Nursing (MSN) or Doctor of Nursing Practice (DNP) in lieu of BSN. Must have two to three years experience in nursing. Excellent written and verbal communication skills as well as mentoring/coaching and organizational skills required. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
