Job Summary
Community Impact Coordinator will identify special populations, social determinants of health for minority communities, overcome language barriers, and coordinate partner meets to best respond to local needs regarding COVID-19 and other health needs.
Responsibilities and Duties
The following duties are the primary responsibility of the Community Impact Coordinator in coordination with other programs.
- Complete CDC Health Disparities work plan objectives (class code 896)
- Coordinate and facilitate ongoing conversations and information-sharing with local rural, disabled, and racial and ethnic minority communities to understand how COVID-19 responses have been successful and where they can improve.
- Establish formal, local advisory groups where feasible and/or
- Strengthen collaboration with established groups that represent local rural, disabled, and racial and ethnic minority communities.
- Assess local support systems’ coordination and effectiveness during the COVID-19 pandemic—specifically related to services to local rural, disabled, and racial and ethnic minority communities. This will include assessment of health systems; access to care usual care; access to COVID-19 testing and vaccination; food security, housing, transportation, and other social determinants of health. Develop and/or update CHIP Priorities to reflect findings, as appropriate.
- Maximize rural, disabled, and racial and ethnic minority communities’ access to usual care by educating individuals about enrollment in Medicaid Expansion.
- Work with community organizations to enhance accessibility to COVID testing, contact tracing, and vaccination.
- Clinical assistance
- Complete Minority Health Initiative work plan objectives (class code 895) for Knox and Cherry counties and expand throughout the district.
- Identification of minority communities is critical to properly identify partners and locations for community listening sessions.
- Identifying the social determinants of health for the minority communities will be identified in the assessment. This will help determine what health indicators are lowest in the community and require intervention or assistance.
- Address the needs of those who do not speak English by creating materials in their language and if needed, contracting with an interpreter. NCDHD has a standing contract with an interpreter.
- Coordinate partner meets to best respond to local minority health needs. NCDHD coordinators will identify potential partners and meet with such entities. At the first meeting, other partners will also be identified and asked to join the group. Partners will meet quarterly to work on assessment, gathering data, strategic planning, implementation of strategies, and evaluation.
- Provide data sets and data analysis to drive the program.
- Grant writing and reporting.
- Assist with accreditation needs. (class code 315)
- Assist with Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) initiatives.
- Other duties as assigned.
Qualifications:
Education and Experience:
Bachelor’s degree in Health Education, Human Services, Health Care, Nursing or related field.
Previous experience in project management, data analysis, and bilingual preferred.
Physical Requirements:
- Prolonged periods sitting or standing at a desk and working on a computer.
- Bending, lifting, or twisting on occasion
- Occasional work in extreme heat or cold
- Must be able to lift at least 30 lbs.
Knowledge, Skills, and Abilities:
- Attention to detail and organizational skills.
- Computer experience including data entry, MS Word, Excel, PowerPoint, Outlook, required.
- Ability to work with minimal supervision.
- Ability to maintain effective working relationships with co-workers, providers and the general public, and community partners.
- Ability to communicate effectively, both orally and in writing.
- Ability to use sound judgment.
- Ability to operate general office equipment.
- Ability to travel locally to provide or participate in activities and training events.
- Ability to work evenings and weekend hours as needed.

Special Conditions:
Evening and weekend hours may be required. Local travel within the service area is required. Occasional travel to other Nebraska communities for purposes of region-wide or statewide collaboration is required. Access to personal transportation is required.
Licensure/Certification:
Valid driver’s license and reliable transportation are required.
