THE POSITION:
Long-Term Acute Care Nurse (LTAC) cares for patients who have a disability or illness in need of extended care. Many of their patients live in long-term care facilities, rehabilitation centers, or nursing homes. The LTAC nurse will assist patients with normal day-to-day tasks like getting dressed and taking baths, but they primarily focus on patient healthcare.
LPN Job Duties:
LTAC nurses have a wide range of responsibilities, which can include:
· Assessing a patient’s condition and planning/implementing the patient’s care plan.
· Ensuring ventilators, monitors and other types of medical equipment function properly.
· Administering intravenous fluids and medications.
· Observing and recording the patient’s vital signs.
· Assisting physicians in performing procedures.
· Ordering diagnostic tests.
· Treating wounds.
· Responding to life-saving situations using nursing standards and treatment protocols; and
· Providing education and support to patients and their family members.
· Preparing patients for procedures by explaining the procedure, what to expect during the procedure, and after care instructions.
· Monitoring patient pain levels and administering pain medications as needed.
· Maintaining accurate records of the nursing care provided to patients including notes about patient behavior, reactions to treatment, and progress notes.
· Performing physical assessments of patients such as taking vital signs, measuring blood pressure, checking weight and height, and recording findings in a medical chart
· Administering medications as prescribed by a doctor or other healthcare professional.
· Providing comfort and emotional support to patients and their families
· Providing wound care and performing other tasks related to managing wounds and infection control.
· Teaching patients how to perform tasks such as bathing, dressing, and using the bathroom with minimal assistance.
· Providing education about disease processes, treatment options, and lifestyle changes that can help people manage their conditions more effectively.
