The Re-Entry Specialist position is responsible for case management responsibilities and ensuring member satisfaction by working collaboratively with cross-functional clinical and peer support teams to assess member needs, evaluate transitional service options, create transition plans, coordinate care, and monitor progress. For adults with recovery and re-entry issues, recovery and success in life depends on good mental and physical health, as well as having satisfying relationships, safe housing, purpose in life, a sense of belonging, and hope for the future. This includes assisting the members with acquiring and utilizing necessary skills to become self-sufficient and to manage and enjoy their lives.
Duties and Responsibilities:
- Participates in the initial assessment and development of individualized service plans, then implements services through excellent case management delivery
- Communicates member needs and status updates to a variety of clinical and peer coach teams and follows-up accordingly
- Partners with members to develop a role for themselves outside of their recovery and re-entry, while focusing on expected outcomes of increased employment, meaningful community activity, independent living status, and positive social networks
- Provides supportive services including but not limited to assistance in maintaining, monitoring, and modifying covered services and case management plans
- Identifies necessary resources to meet members’ needs and help members with obtaining needed resources
- Manages discharge plans upon completion of treatment, as necessary
- Acts as the member’s advocate as it relates to insurance coverage and financial assistance
- Maintains the members’ comprehensive case management records and notes through detailed, accurate, and timely documentation
- Facilitates placement programs and provides resources, including employment programs/resources that are provided by the Transitional Program, Re-entry Specialists, and Peer Coaches
- Facilitates group sessions, as assigned
- Adjusts to fluctuating peaks in member flow, acuity, and other operational demands while maintaining the utmost quality
- Collaborates professionally, with courtesy and tact, when interacting with the leadership team, employees, members, and the community
- Effectively meets any/all outlined Key Performance Indicators (KPIs), ensuring the highest quality and level of member care and engagement is provided
- Receives and actively engages in clinical supervision and oversight each week/month based on BHPP/BHT designation requirements, including completion of all continuing education requirements
- In addition, all other duties as assigned
