The successful Chronic Care Manager:
Graduate of an accredited School of Nursing
Registered by the State of Kansas as a Respiratory Care Practitioner (RCP)
Minimum of two-years experience working in healthcare industry
- Must be empathetic, and possess great communication skills
Essential Functions:
- Perform chronic care management services consistent with each assigned patient's orders and/or established care plan
- Serve as patient advocate by working with each assigned patient to establish and achieve personal goals for health improvement
- Serve as patient educator by providing relevant and actionable information regarding each assigned patient's health conditions
