Job Posting for Client Care Manager - Elder Care at North River Home Care, Inc.
Private duty non-medical agency seeking an experienced elder care professional.
You must have 10 years work experience to be considered for this position, with 5 years in the elder care industry.
Job details:
Developing and implementing care plans for clients
Completing new client assessments (or potential clients), develop relationship with family/client
Act as single point of contact for clients and/or their families
Field supervision of clients and home health aides
Oversight of home health aides assigned to your cases
Coordination with scheduling, human resources, and billing departments
Development of relationships with referral source partners.
Requirements:
You have experience working with the Elderly (5 years).
You are an outstanding communicator with a “can do”, positive attitude.
Strong computer skills with heavy usage of email, texting, and internet based applications.
You are upbeat, warm, friendly and compassionate.
You exude positive energy.
You command respect while being respectful, are thoughtful, intelligent, and caring.
You are detail oriented in care plan development and able to effectively communicate that to many people.
You are extremely focused on providing outstanding service to your clients.
We are extremely focused on customer service throughout our organization; this position is the front line of that effort. We highly value and act upon your input.

Our office is in Norwell , this role is responsible for clients south of Norwell.
