CCBHC Clinical Care Manager
Are you ready to take the first step into your clinical leadership career? The CCBHC Clinical Care Manager fosters collaboration, coordinates clinical services, and supports a multidisciplinary team. We are currently seeking a highly skilled and dedicated individual to join us as! As the CCBHC Clinical Care Manager, you will be a central point of contact, working collaboratively with CCBHC staff and partners to ensure the seamless implementation of clinical services.
Travel required between Derby and St. Johnsbury sites.

RESPONSIBILITIES & SCOPE
- Organizes care activities among different services and providers, and across various facilities.
- Provides clinical supervision to counselors and case managers.
- Oversees data analysis/performance assessment.
- Works with QA/QI team.
- Assists with preparation of required grant reports related to client care.
- Participates actively in the Quality and Utilization Management functions
- Coordinates care across DCOs and other providers by facilitating the intake process, care follow up, and information sharing across all providers.
- Expands relationships and collaborates with other agencies
QUALIFICATIONS
- Master's degree in counseling, social work or related field of mental health.
- Licensure (LCMHC, LCSW or equivalent) preferred.
- Minimum of 5 years of experience working with individuals with severe and persistent mental illness, including experience providing clinical supervision and training.
- Excellent communication and organizational skills.
- Proficiency with Electronic Health Records systems (preferably Credible) and Microsoft Office.

- Competitive Salary $ 52,603-77,604 based on experience.
