Position: Care Coordinator

Summary of Position:
Care Coordination is a service that assists individuals living with Substance Use and Co-Occurring Mental Health Disorders in gaining access to medical, social, educational, and other services. The Care Coordinator is responsible for facilitating access to community resources, assisting the client, family members and other caretakers with developing skills to access other systems supports, assisting with additional skill building activities as identified in the treatment plan, and to coordinate activities such as integrated care team meetings. They will also work to ensure a smooth entry into the NET system of care. Other duties as assigned.
