Position: CUA 7 Outcome Specialist
Job Type/Hours: FT. 40 Hrs. M-F, Schedule is based on the program and client needs.

Summary of Position:
The Outcomes Specialist works to support the Case Manager in helping families achieve Single Case Plan goals and move towards safe, timely case closure. This position involves working closely with all members of the assigned case management team; with children, biological families, and resource families; and with external agencies and partners. The Outcomes Specialist will focus on placement cases; include direct work with families to address safety threats and diminished protective capacities, as well as work with external partners and systems. The Outcomes Specialist will carry a caseload not to exceed 15 placement cases or court-involved in-home cases with special emphasis on tasks related to achieving reunification or other permanency.
