NurseNow Staffing is urgently hiring Hospice Home Health Nurses for the Oshkosh, WI area
Contract:
- 13-weeks
- Full-time, Mon - Fri Days
- 40 Hours Guaranteed per week
- Strong possibility for extensions
Setting:
- Field staff all work in field after training.
- Most of our patients will be in Oshkosh, WI
- Exact coverage area to be discussed during the interview
Additional Job Responsibilities related to Home Hospital:
- Responsible for Caseloads of Patients in specialty areas.
- Conducting initial assessments to obtain detailed physical, and environmental data.
- Establishing a plan of care in regulation with physician orders
- Advises and consults with family
- Provides interventions and ongoing assessments of patient care
- Completes documentation according to regulations
- Must be able to provide care to CHF patients and identify critical care elements within each patient's specific care plan

Requirements
- Active WI RN license
- Active BLS
- At least 1 year of Home Health & Hospice Experience, recent preferred
- Technically Savvy, Strong Computer and Phone Skills
- Must be able to lift up to 20lbs
- Must be willing to travel
- Strong EPIC experience
- Reliable Transportation
- Active Driver's License
- Strong Communication Skills
