Oak Brook Care is in need of an Memory Care Assistant to join our skilled nursing facility in Oak Brook, IL:

Job type: Full-time
Hours: 8am-4pm or 12pm-8pm
Pay: $12.00 - $14.00/hr.
Start: Immediately
Position Summary:
- To execute the activity program as directed by the Director of Activity and to encourage resident participation.
Qualifications:
- High school education or equivalent
- Good oral and written skills.
Responsibilities and Duties:
- Under the supervision of the Activity Director, to direct and execute a well balanced activity program. One activity assistant in an activity program once it has started.
- Announce activity program 30 minutes before the program will begin and then repeat announcement.
- Invite residents to attend, escort and transport residents to activities as needed or requested by the residents.
- Reach residents on an individual level in order to motivate them to attend and participate in activities.
- Conduct evening and weekend programs at the direction of the Director of Activity.
- Provide 1:1 visits with residents as needed or assigned. Provide regular 1:1 visits with those residents in isolation, using proper isolation technique.
- Daily recording of resident’s participation/response in activities. Regular recording of resident’s participation/response during 1:1 visits.
- Charting resident’s progress monthly/quarterly as needed (assignment of specific staff members). Completing reassessments as needed (assignment of specific staff members).
- Responsible for the daily preparation of materials and supplies needed for the days program and for informing Director of Activity of supplies needed.
- Contacting and familiarizing new residents with the activity opportunities available to them.
- Communication of the activity schedule to residents - - updating Bulletin Boards at all times.
- Safety of the residents: proper transfer techniques, transporting resident, use of restraints, special diets, serving refreshments safely, and reporting of noteworthy conditions or situations.
- Completing specific duties usually assigned to other staff members when that staff member is not at work.
- Assist at meal times transporting, transferring, passing trays, and removal of trays.
- At all times, observe for resident’s privacy, dignity and comfort.
- Ensures that State, Federal and JCAHO standards are maintained to ensure quality of services and resident care.
- Will only access the minimum PHI necessary to accomplish my job.
Physical Requirements:
- Good physical and mental health.
- Ability to lift and transport equipment and residents.
- Able to be on feet for extended periods of time.
Special Requirements:
- Understanding, patience, tact and courtesy in dealing with employees, residents and visitors.
- Ability to cooperate with other health professionals and to coordinate efforts toward maximum patient care.
- Special interest in the geriatric population
