Title: Triage Registered Nurse (RN)
OakLeaf Clinics – Eau Claire Medical Clinic has an exciting opportunity to join our expanding team as a Full- Time Triage Registered Nurse.
OakLeaf Clinics is dedicated to providing our patients with compassion, trust, and a lifetime of individualized care. Our healthcare team consists of physicians, nurse practitioners, physician assistants, dietitians, nurses, respiratory therapists, and medical assistants working in concert with laboratory/imaging services to offer individualized care to the Chippewa Valley.
Position description
As the Triage Nurse you will provide professional nursing assessments, prioritize treatments according to the urgency of need, and initiate medical care to patients arriving at the emergency department. This position is Full-time working Monday, Tuesday, Wednesday, Thursday and Friday for 40 hours per week and is located at our Eau Claire Medical Clinic at 3802 Oakwood Mall Drive Eau Claire, WI 54701 with possible travel to 855 Lakeland Drive
Chippewa Falls, WI 54729.
Responsibilities
· Provide patient centered nursing care in outpatient clinic according to established scope of practice guidelines and standards of care
· Develop, coordinate, and implement patient plan of care in collaboration with health care team
· Use nursing process to provide ongoing care (assess, plan, intervention, and evaluation)
· Recognize and respond to changes in patient's status and need for care
· Determine priorities of patient care based on essential patient needs
· Provide triage and consultation to patient, family, and/or significant others according to protocol and organization guidelines
· Communicate with physicians and other members of the health care team and documents accurate and timely information regarding patient conditions
· Prepare and room patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e., chief complaint, allergies, medications)
· Perform laboratory procedures (i.e., strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines
· Administer medication, under physician/provider orders, including: immunizations, antibiotics, narcotic pain medications, vitamins, topical agents, insulin, blood thinners, and allergy antigens
· Assess and provide basic to complex patient education and counseling on the performance of tasks, skills, treatment and self-care integrating health promotion and preventive aspects of care
· Follow through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services.
· Refer questions to physician/provider and relays information back to patient as directed.
· Perform advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), spirometry, Holter monitor application, casting, complex wound care, fetal heart tone monitoring, Hickman/port-a-cath/PICC line, central line care, conscious sedation, anti-coagulation clinic operation, etc., with demonstrated competency and under clinical supervision
Qualifications
· Graduate of an accredited school of nursing
· Current State of Wisconsin Registered Nurse License or Nurse Compact License
· Basic Life Support (BLS) certification, Advanced Cardiac Life Support (ACLS) awarded by the American Heart Association (AHA) within three months of hirE
Previous clinic RN experience required
· Excellent communication skills (verbal and written)
· Experience working in a team environment
· Ability to adapt to unpredictable situations within the work setting
· Demonstrated leadership and excellent critical thinking skills
· Epic experience

Major activities of this job include frequent sitting for extended periods of time, some walking or standing, and light physical effort performed on a level surface. Some carrying, lifting, kneeling, bending, reaching, and squatting are involved in the medical office. Use a computer and telephone and employ writing and organizational skills. Manual dexterity, mobility and good vision that includes near acuity, depth perception and accommodation are physical requirements necessary for technical work.
