Company Intro: Ocean Health Initiatives, Inc. (OHI) is a Federally Qualified Health Center (FQHC) dedicated to providing quality, accessible and comprehensive primary health care to the residents of Ocean and Monmouth County; regardless of economic status.
Our Health Center Locations: Brick, Freehold, Lakewood, Little Egg Harbor, Manahawkin, Manchester and Toms River; with our school-based Wellness Programs located within the Clifton Ave Grade School, Lakewood; and Lakewood High School.
OHI services include family and internal medicine, pediatrics, behavioral health, nutrition, OB/GYN women’s health, dental, family planning, specialty care, pharmacies, PrEP, and a STI clinic.
Hours: Full-time hours are M-F 8:00am- 4:30pm with rotating evening/Saturday shifts (subject to seasonal changes and business/site needs)
Position Summary
Reporting to the Vice President of Quality Assurance and Enabling Services the Registered Dietitian in collaboration with Ocean Health Initiatives, Inc. (OHI), staff physicians, will be responsible for planning therapeutic diets, educating individual patients about proper nutrition, diet, and food selections and disseminates health education materials for patients and their families.
Responsibilities
- Conduct nutrition assessments of patients referred for this service by healthcare providers.
- Maintains nutritional care plans, reviews medical records, laboratory reports and other nutritional care, documents findings and attends medical conferences to collaborate with the medical team.
- Develops and distributes dietary/nutritional educational material.
- Develops menus for therapeutic diets based on medical and physical condition, life style and culture, and nutritional needs of patients.
- Instructs patients and their families in nutrition principles, dietary plans, food selection and preparation, and nutrition and weight management.
- Functions as a member of a multi-disciplinary health care team.
- Assists with the development, implementation, and monitoring of clinic policies, procedures and protocols, as needed to achieve OHI goals and objectives.
- Assists with the development, implementation, and monitoring of OHI Performance Improvement program.
- Attends and actively participates in departmental and medical staff meetings as required.
- Supports health center colleagues with internal referrals when clinically appropriate.
- Collects patient information and records patient information, such as medical history and reports examination results.
- Conducts ongoing education programs and activities in nutrition and weight management for patients, families, and OHI staff.
- Provides consultation, guidance and directions to medical support staff to ensure continuity and quality of care services.
- Effectively and efficiently completes all paperwork requirements for billing and medical records compliance.
- Renders nutritional assistance to patients engaged in specialized health promotion and disease.
- Develops nutritional support group as well as provides ongoing support to patient in individual counseling sessions to enable patients to change behaviors which affects their nutritional status and health conditions.
- Advises patients on good health practices.
- Evaluates how patients respond to their diets. Check follow-up that they are following their diet at home.
- Develops/implements nutrition in-services for OHI staff.
- Plans, develops, and executes patient group classes on topics such as diabetes, cardiovascular disease prevention, or other organizational needs.
- Actively enrolls patients in Diabetes self-management education and support (DSMES) and maintains accreditation as the Program Coordinator for the Diabetes Education Accreditation Program through the Association of Diabetes Care & Education Specialists.
- Actively engages patients in the importance of nutritional services and drives encounter productivity.
- Uses effective clinical judgment in referring patients and receiving referrals. Develops and implements goals and objectives to ensure high quality, cost effective clinical care.
- Ensures all patient charts are completed within 24 hours of the patient visit and the charting is accurate to support clinical and documentation requirements via Electronic Health System (EHS).
- Plans and implements policies, procedures, and systems to achieve stated goals and to ensure compliance with regulatory and third-party agencies and mandates.
- Active participant in OHI's Performance Improvement Process, Departmental meetings, and other OHI meetings as scheduled.
- Provides the CMO with written reports as directed.
- Willingness to work evenings and weekends as required.
- Ability to work in a variety of settings such as the traditional office, mobile medical van, etc.
- Ability to effectively communicate orally with diverse individuals and groups.
- Ability to work with patients of different cultures.
- Ability to work within a team setting and to relate to the special needs of the patient population.
- Provides consultation, guidance and direction to clinical support staff to ensure continuity and quality of services and care.

- Presents information to patients at an educational level consistent with their understanding.
- Listens attentively to patient complaints and resolves or refers to appropriate individuals.

- Adheres to the American Medical Association Code of Professional Ethics.
- Keeps all matters related to the organization confidential in compliance with confidentiality policy.
- Works collaboratively with all members of the Senior Management and Management Teams to function as a cohesive and professional unit.
- Works collaboratively with all members of the Senior Management and Management Teams to resolve departmental and organizational challenges expeditiously, economically, professionally and with minimum disruption to the practice.
- Leads and manages staff by professional example.
- Constantly strives to achieve excellence in day-to-day operations of respective department.
- Completes all mandatory in-services.
- Adheres to Corporate Compliance policies.
- Is highly accountable for one's own productivity. In order to maintain transparency, all projects must be represented clearly on a project board in Monday.com. Responsible for ensuring the board is always updated and accurate and that information is accessible to those that need it.
- Adheres to Corporate Compliance policies.
- Maintains a passing monthly scorecard threshold at 80%.

Education/Experience/Licensure
- Bachelor of Science Degree in Nutrition.
- Active State of New Jersey Registered Dietician License.
- Three to five years of experience at Registered Dietician in a healthcare setting.
- Bi-Lingual (English-Spanish) strongly preferred.
- Basic Life Support for providers through the American Heart Association is required.
- Electronic Medical Record (EMR) experience is preferred.
- Proficiency in Microsoft Office 365 is required.
