This job assists with the medication reconciliation process by ensuring an accurate and complete home medication list is documented for patients admitted to Ochsner Medical Center. Communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain an accurate and complete medication history for patients; identify discrepancies or problems with medication history information and notify a pharmacist to seek clarification or resolution; ensure capture of appropriate patient information necessary to generate safe and effective medication orders (i.e. allergies, height, weight); ensure capture of prescription benefit information and document the home medication list in the electronic health record (EHR). Education Required – High School diploma or equivalent Successful completion of a pharmacy technician training program Work Experience Required – 2 years of experience as a Pharmacy Technician Prior experience taking patient medication histories preferred Certifications Required - Registered with the Louisiana Board of Pharmacy and in possession of a work permit. One year of college or completion of a para-professional pharmacy training program, or Certified Pharmacy Technician Licensure or National Certification Knowledge Skills and Abilities Must have computer skills and dexterity required for data entry and retrieval of patient information. Must have working knowledge of pharmaceutical mathematics Must have working knowledge of sterile technique and demonstrate aseptic technique proficiency Must have working knowledge of safety guidelines for handling chemotherapy Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process. Must be proficient with Windows-style applications, various software packages specific to role and keyboard. Skills to operate standard pharmacy and office equipment. Strong interpersonal skills. Job Duties Accurately obtains and clarifies patients' home/chronic medication list by contacting community pharmacies, physician offices and/or extended care facilities, interviewing patient and/or family members to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and complimentary & alternative medications in a timely manner Utilizes the electronic health record to input patients’ home/chronic medication lists for the purposes of medication reconciliation under the supervision of a pharmacist Communicates discrepancies identified via the medication history process with the pharmacist and makes clarifications with the patient and prescribers as needed Accurately collects patients’ prescription benefit information Appropriately directs, triages or communicates to the appropriate caregivers or staff all questions or inquiries from patient or families that are outside the scope of taking the medication history Provides high quality customer service to patients, family members, and care team members Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style. Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible. Mechanical lifting devices (carts, dollies, etc.) or team lifts should be utilized. Must be able to stoop, bend, reach and grab with arms and hands, manual dexterity. Duties performed routinely require exposure to blood, body fluid and tissue. The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases.
