Omni Therapy, Inc. is looking to add Care Coordinators to our growing team. This position is a remote opportunity, so you can work from home 100% of the time!
Omni Therapy Inc. is committed to facilitating the best in-home therapeutic services to patients in their homes throughout Southern California. Remote Case Managers report to and act as a liaison between clinicians and home health agency partners. The Remote Case Manager would be responsible for patient chart oversight, ensuring that clinicians are treating patients in a timely manner as well as with the prescribed frequency in addition to submitting the appropriate documentation.

Responsibilities Include:
- Facilitating care delivery and timely discharge or transfer of patients.
- Ensuring that appropriate type and frequency of care are performed by clinicians.
- Ability to multi-task with phone calls, text messages, and emails.
- Providing outstanding customer service to home health agencies and clinicians.
Requirements:
- Must be a California resident and currently live in the state of California.
- Minimum of 2 years experience as a home health case manager or care coordinator is required.
- 3 positive references verifying professional background.
- Availability to work full time, Mon - Fri from 9am - 5:30pm.
