Professional Opportunity Details:

Omni Therapy is looking for a contract Registered Nurse (RN) to conduct SOC visits with patients in their homes in the Hidden Hills CA area.

Previous SOC Oasis experience is NOT required. Omni Therapy will fully train you! 

The overall purpose of the Home Health RN for SOC position is to conduct start of care Oasis visits to patients in their home who have been approved for home health based care.  

Essential Functions:

• All OASIS documentation will be completed and synched within 1 business day of the visit.
• Skills in communication, documentation, assessment, and home health methods and techniques.
• Completes all necessary and supportive documentation-assessments, evaluations, visit notes and discharges, in accordance with state and federal regulations.
• Responsible for assuring patient safety while visiting in the home.

Must-Have:

•  A valid California RN license
•  Active driver’s license, auto insurance, CPR, professional liability insurance, current physical & TB test.
