job Summary: We are seeking a compassionate and detail-oriented Registered Nurse to conduct home visits for adult and foster care patients. The ideal candidate will assess, monitor, and evaluate the health conditions of patients in their home environments, providing professional medical care and support to ensure optimal health outcomes.
Key Responsibilities:
- Perform comprehensive assessments of patients’ health conditions during home visits.
- Develop and implement personalized care plans based on patient evaluations.
- Monitor vital signs, administer medications, and provide wound care as needed.
- Educate patients and their families about health management and treatment plans.
- Collaborate with other healthcare professionals to coordinate patient care and referrals.
- Maintain accurate and up-to-date medical records and documentation.
- Adhere to safety protocols and maintain confidentiality of patient information.
Qualifications:
- Valid Registered Nurse (RN) license.
- Minimum of 1 year of nursing experience, with experience in adult or foster care preferred.
- Strong clinical assessment skills and ability to work independently.
- Excellent communication and interpersonal skills.
- Proficient in using electronic health records (EHR) systems.
- Valid driver’s license and reliable transportation for home visits.
Working Conditions:
