OneFifteen is a not-for-profit healthcare ecosystem dedicated to the full and sustained recovery of people suffering from substance use disorders. We have developed a tech-enabled system of care that offers a treatment center, rehabilitation housing, and wrap-around services, all on one state-of-the-art campus. OneFifteen will deploy a learning healthcare system to advance our understanding of addiction medicine and improved strategies to promote sustained recovery. Our ultimate goal is to support individuals to live healthy, fulfilling lives free of addiction. As such, we will focus on a patient-centered experience and ensure a patient’s whole health needs are fully met, including biopsychosocial needs.

OneFifteen is launching a scalable, Comprehensive Hybrid Care model to increase access to its tech-enabled care for individuals in Franklin County, Ohio. This evidence-based care program includes a virtual substance use disorder medicine clinic, a hybrid (virtual or in-person) care advocate team, and the tech-enabled platform to deliver care and support participants.

Description

The Care Advocate Team Lead is a licensed social worker or counselor who provides direct service to participants and provides training and supervision to the Care Advocates. The Team Lead is a trusted team member who has extensive knowledge of the community served and of community resources to service at-risk populations. The position acts as the main connection for participants throughout their recovery journey, keeping participants engaged in their care, addressing social determinants of health, and ensuring referrals and resources are provided in a timely fashion to participants. The position acts as a liaison between community resources and the participant and/or family to improve service delivery quality and cultural competence. The Team Lead takes a multi-disciplinary approach to service delivery and coordinates with treatment partners, community resources, and social service agencies; ensures compliance and fidelity to the program policy and protocols. The Care Advocate Team Lead uses OneFifteen’s measurement informed care (MIC) program by reviewing data and recovery domain scores, then uses these scores to guide patients on their recovery journey. The Care Advocate Team Lead also schedules regular meetings with participants and partner agencies to maintain communication and collaboration about services; works directly with all program members to maintain and retain project data and materials as required; supports the completion and submission of progress and other required reports; develops and maintains strong relationships with stakeholders and community partners.

This is a full-time (40 hours per week) position located in Dublin, OH.

Responsibilities

  • Provide direct supervision and training to the Franklin County Care Advocates, which also includes review of documentation and signing off notes.
  • Participate in design and implement solutions to enhance processes and coordinate with external partners.
  • Effectively communicate technical concepts to non-technical internal and external partners, and work effectively between technical and non-technical teams.
  • Responsible for establishing trusting relationships with participants and their families while providing navigation and linkage to needed community resources.
  • Assess participants' psychosocial needs and help participants remove barriers to recovery.
  • Manage an assigned caseload of participants by:
  • Review clinical assessments in healthcare software tools (OneFifteen Care Team Platform) and discuss results with participants.
  • Provide referrals for services to community agencies as appropriate.
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with participants and family members.
  • Review and support a participant’s recovery progress through the use of validated assessments and technology tools designed to support personalized recovery journeys.
  • Provide education about the program, including but not limited to education about medications for substance use disorders.
  • Follow–up with participants via telehealth (video or chat), telecommunication or in-person visits in the home or other settings where participants can be found.
  • Triage participants medical and behavioral health needs and collaborate with multidisciplinary providers for appropriate patient placement.
  • Help participants set personal goals and ensure direct access to resources to support goal attainment.
  • Help participants connect with transportation resources and provide appointment reminders in special circumstances.
  • Exhibit excellent working relationships with participants, family members and all community partners.
  • Responsible for providing consistent communication with the participant, family members, and other service providers, and reports clearly describe progress.
  • Act as a liaison between the participant and family members and community service agencies.
  • Provide timely written documentation and update participants’ recovery plan as required.
  • Work cohesively with all team members.
  • Attend regular staff meetings, training, and partner and community meetings as required.
  • Support participants in the adoption of technological solutions.
  • Meet participants both in person and virtually.
  • Establish trusted advisor relationships with participants and OneFifteen users, have a firm grasp of the details of their accounts, and drive adoption and utilization of OneFifteen solutions.
  • Travel is required to locations within Franklin County. Some travel may be required outside of the county.

Minimum Qualifications:

  • Bachelor Degree in social work or counseling.
  • Licensure in the State of Ohio LSW or LPC.
  • One-three years of program implementation across multidisciplinary teams.
  • One-three years of direct supervisory experience.
  • Experience working with individuals with mental health and substance use disorders.
  • Experience with crisis intervention and management.
  • Extensive knowledge about community resources appropriate to the needs of participants served and family members.
  • Ability to work with many diverse people, including family members.
  • Demonstrates cultural competency.
  • Demonstrated knowledge of technology, with a preference towards comfort with healthcare software (e.g., EHRs, telehealth software, and mobile applications).
  • Ability to make suggestions on workflow or system efficiency and effectiveness.
  • Ability to work independently and be self-directed and flexible.
  • Ability to perform functions with minimal supervision.
  • Ability to work at a high-volume level of accuracy.
  • Excellent verbal and written communication skills and the ability to simplify complex concepts for varying levels of audiences.
  • Excellent customer service and people skills and ability to communicate effectively in written format and oral presentations.
  • Excellent organizational skills and very detail-oriented.
  • Experience managing relationships with multiple internal and external stakeholders and partner agencies.
  • A strong desire to work in a fast-paced, evolving startup environment.

Preferred Qualifications:

  • Masters Degree in social work or counseling.
  • Licensure in the State of Ohio LISW, LPCC, LISW-S, LPCC-S.
  • Three-five years of program development and implementation across multidisciplinary teams.
  • Three-five years of direct supervisory experience.
  • Experience with implementing virtual care programs (virtual supervision) and asynchronous communication.

Pay: $56,000.00 - $64,000.00 per year
