Oneida Health is an independent 101-bed acute care community hospital and a 160-bed extended-care facility (ECF) and short-term rehab facility licensed by the State of New York and operated by Oneida Health Systems, Inc., a New York not-for-profit corporation. The hospital is Joint Commission accredited.

Summary:

Under the general supervision and direction of the Nurse Manager, or Clinical Supervisor develops a nursing diagnosis according to the age and developmental level of the patient, and treats human responses to actual or potential health problems, utilizing the nursing process in accordance with the standards established by the State Nurse Practice Act, Code of Ethics for Nurses, ANA Standards of Care and the mission, philosophy, policies and procedures of Oneida Health Care.

Essential Job Responsibilities:

  • Applies the nursing process to direct patient care by observing, collecting and recording data related to adult and geriatric patient’s biophysical, psychosocial, environmental, and functional ability as well as educational and discharge planning needs.

1 develops, documents and modifies a plan of care based on patients’ assessments, diagnosis, needs, age and developmental level.

2 discharge planning reflects collaboration with other disciplines

3 patient commitment will be a priority to allow patients to be heard and expectation of their care met.

  • Reassesses patients at intervals throughout the shift based on diagnosis, condition, needs, age and developmental level and documents findings.
  • Recognizes and assesses emergent patient care situations, directs and documents care given to include patient/family response.
  • Assign/ delegate patient care/duties based on staff skill, knowledge, patient acuity and available resources with appropriate follow-up.
  • Takes proactive approach to identifying and managing unit situations and addressing customer (patient, family, co-workers, and department needs).
  • Demonstrates skill in computer operations including documentation, electronic data entry, and retrieval.
  • Demonstrates responsibility and accountability for own professional practice consistent with healthcare and organizational standards:

1 identifies own educational needs and participates in mandatory education and in-services

2 keeps informed of current developments within own area of practice

(i.e. attends staff meetings and committees etc.).

  • Promotes and maintains overall cleanliness of the unit.

1 ensures proper functioning equipment on unit and supplies needed for patient care are available and in good working order.

2 emergency equipment checked and maintained per policy.

  • Utilizes appropriate communication techniques based on knowledge of normal age-specific development during interactions with/care of adult and geriatric patient/family; demonstrates understanding and respect for cultural values, beliefs and practices.
  • Provides for a safe environment and appropriate care for the adult and geriatric patient utilizing age-specific developmental criteria.
  • Implements safe and therapeutic interventions in a timely manner in conjunction with the physician, patient, family and other members of the interdisciplinary team by setting priorities for care based on the adult and geriatric patient’s needs and department policies relating to:

1 medication administration

2 IV therapy

3 procedures necessary for the patients condition

4 labels specimens and completes order prior to sending to Lab

5 point of care testing as directed and ordered.

  • Assesses the patients’/family learning needs at the time of admission and throughout the hospital stay.

1 identifies common health concerns for the adult and geriatric patients

2 implements teaching based on identified needs and documents appropriately

3 involves the patient/family in care in ways that are appropriate for the patient’s age, developmental level and cultural/spiritual preferences

  • Uses accepted Infection Prevention techniques to prevent and reduce the risk of acquiring and/or transmitting infections and manage all body fluids from all persons at all times. (Failure to comply shall constitute unprofessional conduct in accordance with NY State Education Dept. Rules Section 29.2 for all licensed health care professionals and all those involved with patients directly or indirectly.)

1 Performs hand hygiene with the appropriate product (i.e. foam vs. soap/water) at all appropriate times/situations.

2 Wears appropriate protective gloves at all times when touching blood, saliva, other body fluids or secretions, mucous membranes, non-intact skin, blood-soiled items or bodily fluid-soiled items, contaminated surfaces, and sterile body areas

3 Washes hands after removing gloves.

4 Discards gloves used following treatment of a patient and applies new gloves if torn or damaged during treatment of a patient

  • Correctly identifies patients prior to performing interventions or care.

FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:

  • Good knowledge of nursing principles and techniques and their relation to medical and surgical practices and skill in their application;
  • Working knowledge of material, medical, hospital dietetics, sanitation, and personal hygiene;
  • Skill in the application of nursing techniques and practices;
  • Ability to understand and follow technical oral and written instructions;
  • Ability to keep records and make reports;
  • Ability to get along well with patients, physicians, and others;
  • Ability to carry out successfully the therapeutic measures prescribed;
  • Ability to plan and supervise the work of subprofessional and non-technical personnel;
  • Good observation skills; mental alertness; firmness; initiative; cheerfulness; patience; sympathetic attitude toward patients and residents; physical condition commensurate with the demands of the position.

MINIMUM QUALIFICATIONS

  • Graduation from a school of nursing approved by the State Education Department from a course approved by such department as qualifying for Registered Professional Nurse
  • Valid NYS RN license
  • ACLS and BLS Certification
