As the LPN of Central Admissions, you will facilitate the timely and appropriate initiation of home health, hospice, and infusion pharmacy services for all referred patients. This is a remote Monday-Friday position with weekend and holiday rotation.

This position may be remote after orientation. You will have to live within an hour of Cleveland due to technical support and requirement for in-person meetings.
Responsibilities:
- Assesses the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
- Coordinates initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
- Incorporates clinical and financial knowledge with referral process and recommends appropriate alternative approaches when appropriate.
- Communicates the home care needs with scheduling, managers, reimbursement, and other parties involved in the home care services process.
- Serves as the CCHCS information specialist; understands other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.

Education:
- Graduate of an accredited practical nursing program.
Certifications:
- Current licensure as an Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
Complexity of Work:
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
Work Experience:
- One year clinical experience in home care strongly preferred.
Physical Requirements:
- Requires sitting, standing and walking for extended periods of time. Requires manual dexterity to perform work on a PC.
- Requires good visual acuity through normal or corrected vision and must be able to hear typical conversation levels through normal or corrected hearing.
- Must be able to carry / transport office equipment including a desktop, laptop and monitor to home / work environment.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
