ESSENTIAL DUTIES AND RESPONSIBILITIES
The Care Navigator is accountable for assuring a high standard of quality and care management support for the value-based care (VBC) contracts with which OnPoint participates. This position is responsible for having a full understanding of the quality measure specifications and payment methodologies associated with the various VBC programs. The Care Navigator will be working to optimize performance in VBC contracts through the attestation of services rendered via payer portals, in collaboration with quality payer program representatives to achieve measurable outcomes.
This position is onsite at the Highlands Ranch Corporate Office.
DUTIES INCLUDE BUT NOT LIMITED TO:
- Conduct chart reviews, data entry, documentation, and attestation submissions for value-based care gap closures.
- Conduct outbound calls to patients recently discharged from hospital to ER to ensure appropriate follow-up with primary care providers.
- Collaborate with the Population Health Team to identify barriers and develop solutions to meet quality metric targets, reduce costs and improve health outcomes for patients.
- Support clinic initiatives for care delivery interventions and outreach patients with gaps in care, as needed.
- Communicate quality metric notifications to providers and practice teams via EMR.
- Conduct outbounds calls to pharmacies and/or patients regarding medication adherence.
- Outreach specialists for consult notes, as needed.
- Review payer patient lists to supply clinics with the most up to date information on patients with open gaps.
- Manage daily distribution of the transitional care management patient file to clinics and upload monthly patient attribution to CORHIO.
- When the need arises, will perform other duties as assigned by supervisor.
Qualifications
Licensure/Certification/Education/Experience
- Experience in Population Health/Quality, care management/coordination, primary care or other related field.
- Commensurate experience in public health, social services or other health care field may take the place of this requirement.
- Competence in the ability to triage patients over the phone.
- LPN License or Medical Assistant Certificate preferred.
- Strong computer skills, particularly related to Microsoft applications (Word and Excel).
- Working knowledge of health information technology
- Comfort with exploring and learning to use new or unfamiliar applications or databases.
- Experience with Athena EMR preferred.
- Experience with Value-Based Care payer portals.
