What are the responsibilities and job description for the MDS Coordinator RN position at OnPointe Dallas Transitional Care?
The RN MDS Coordinator promotes the physical well-being of residents by performing assessments, analyzing data, and formulating individualized care plans with the support of the medical team. The MDS Coordinator ensures accountability and compliance.
- Complete accurate and timely MDS, CAA, and plan of care in accordance with the RAI guidelines for all reside
- Determine potential RUGs and expense associated with admission
- Complete required documentation in an accurate and timely manner
- Monitor Case Mix Index (CMI) scores, looking for potential risks or changes that may affect reimbursement
- Provide resident status updates as needed to appropriate personnel and family members
- Maintain current t knowledge of Medicare and Medicaid criteria, serving as a resource for staff and communicate changes
- Serves as a resource for complex admissions
- Monitor assessment schedules and nursing documentation ensuring accuracy and compliance
- Adheres to facility policies and procedures and participates in facility quality improvement and safety programs
- Demonstrate respect and compassion in every interaction between co-workers, residents, and vendors adhering to organization core values and mission
- Perform other duties as assigned
Education and Experience:
- Must possess a current license to practice as a Registered Nurse in the state of Texas or other compact state
- Minimum, three (3) years of nursing experience in a similar setting serving the same age or population preferred
- Two (2) years MDS experience preferred

- Must be able to sit at a desk and working on a computer for a prolonged period of time
- Must be able to lift up to 50 pounds at times with frequent lifting, carry, bending, and stooping
- Must be able to utilize verbal, sight, and hearing senses to communicate to patients and the interdisciplinary team
- Must be able to use assistive devices such as a Hoyer lift, wheel chair, and other safe patient handling transferring tools
- Must be able to adhere to infection control practices wearing protective gown and gloves when required
- Subject to exposure to infectious waste, disease, viruses, conditions, needles, and sharps
- Must be posses the ability to tactfully communicate with irate patients, elderly population, and state/federal agencies
