Reports To: Clinic Director
Job Purpose: The Medication Assistance Coordinator is responsible for facilitating access to prescribed medications for patients, ensuring they receive the necessary pharmaceutical assistance programs, and providing support to healthcare professionals in managing medication-related challenges.
Key Responsibilities:
Patient Assessment and Consultation:
- Conduct assessments to determine patients' eligibility for pharmaceutical assistance programs.
- Provide consultation to patients regarding their prescribed medications, including dosage, administration, and potential side effects.
Program Enrollment and Application Management:
- Assist patients in completing application forms for pharmaceutical assistance programs, ensuring accurate and timely submissions.
- Monitor the status of applications, follow up with patients and program providers, and resolve any issues or discrepancies.
Insurance Verification and Coordination:
- Verify patients' insurance coverage for prescribed medications and identify any restrictions or limitations.
- Collaborate with insurance companies, healthcare providers, and pharmacies to resolve coverage-related issues.
Financial Assistance and Resource Referral:
- Identify alternative sources of financial assistance for patients who do not qualify for pharmaceutical assistance programs.
- Refer patients to relevant social service agencies, foundations, or charitable organizations for additional support.
Medication Education and Adherence Support:
- Educate patients on proper medication usage, potential side effects, and the importance of adherence to prescribed regimens.
- Monitor and track patients' medication adherence, providing reminders and reinforcement as needed.
Documentation and Reporting:
- Maintain accurate and confidential records of patient interactions, program applications, and medication assistance activities.
- Generate reports detailing program utilization, patient outcomes, and any notable trends.
Collaboration and Communication:
- Work closely with healthcare providers, pharmacists, case managers, and other members of the healthcare team to ensure coordinated care.
- Communicate effectively with patients, providing clear explanations and addressing any concerns or questions.
Compliance and Regulatory Adherence:
- Stay informed about relevant laws, regulations, and industry standards related to medication assistance programs.
- Ensure all activities and interactions comply with legal and ethical guidelines.
Qualifications:
- Associate degree or equivalent experience in healthcare-related field (e.g., Nursing, Pharmacy, Social Work) preferred.
- Knowledge of pharmaceutical assistance programs, insurance systems, and healthcare access resources.
- Strong communication and interpersonal skills.
- Attention to detail and proficiency in documentation.
- Empathy and sensitivity when dealing with patients facing health-related challenges.
- Ability to work independently and as part of a multidisciplinary team.
- Prior experience in healthcare coordination, patient advocacy, or similar roles preferred.
- Familiarity with electronic health record (EHR) systems and relevant software applications.
