Objective
Under the direction of the Administrator and Director of Nursing, the Medical Records Clerk maintains the medical records in accordance with State and Federal Regulations as well as professional standards of practice and facility policies and procedures to ensure complete, timely, and accurate medical records. The Medical Records Clerk is responsible for managing all aspects of the medical patient record from admission to discharge. Responsible to order, distribute, and maintain supplies and equipment for continuous and timely resident care related services.
Responsibilities:
Monitor physician visits and documentation
Produces record on request according to corporate guidelines
Manages orders
Performs Quality Assurance Performance Improvement (QAPI) checks for completeness and accuracy
Provides nursing stations with proper chart and documentation forms.
Maintains daily census listing, daily room assignment listing, physician/patient list, and
admission/discharge/transfer register.
Prepares requests for medical information as directed by Director of Nursing or Administrator
Manages patients clinical record from admission to discharge
Processes telephone orders including mailing, maintaining telephone order log book and filing upon return
Promotes guidelines for documenting, storing and retrieving information
Maintains confidentiality of medical records
Orders supplies, and forms as needed
Takes inventory of and rotates stocked items daily and maintains monthly inventory of all nursing supplies.
Orders and maintains inventory of supplies and fills requests according to procedures.
Places central supply and vendor orders on a weekly basis, to maintain adequate inventory as demonstrated by requisition order; Insures timely shipping of orders.
Forwards purchase orders to Accounts Payable Bookkeeper.
Unpacks and checks supplies upon receipt from vendors or other suppliers, and reports discrepancies in orders to
Administrator; Packs supplies which need to be returned or sent-out.
Places charge stickers or charge slips on supplies.
Initiates resident charges according to written policies and procedures.
Receives, sorts, and cleans equipment in accordance with decontamination policies.
Orders, maintains, and assembles equipment and puts them in proper position.
Maintains budget as recorded on budget sheet; Reports over budgeting to Administrator
Insures that special equipment is in good working order.
Maintains accurate records of resident payor status i.e.: Medicare, Medicaid, Skilled or Private; Maintains accurate records of resident requirements for ASM supplies; Notifies ASM of changes in resident status i.e.: death, discharge, change in Doctor’s order.
Qualifications
High school diploma required; Associates degree in Medical Record Technology preferred; Completion of classes in medical terminology and knowledge of HIPPA.
One to two years previous experience in medical records; previous typing and office experience preferred.
Certification in Central Supply from an accredited school, preferred; Ability to follow written and oral instructions; Knowledge of Medical terminology.
One year of previous work-related experience preferred; Familiarity with supplies handled and computer literacy required.
Certified Medication Aide.
