Responsibilities
- Acting in the scope of the Illinois Nurse Practice Act.
- Support & manage the Weekly Medication Management needs for our Home Nursing Services Clients, including:

- Reconcile medication orders issued by duty licensed prescribing practitioners for Clients.
- Place medications in the designated weekly medication cassette for each day and time specified on the Physician Order & Care Plan.
- Maintain medications securely in Client’s medication cassettes & storage boxes.
- Place refill orders with pharmacies and arrange for delivery of medications. Complete follow ups when needed to ensure all medication are received on time prior to next weekly fill, maintaining a sufficient quantity of Client medications.
- Communicate & document record of medication orders, medication refills & deliveries accurately accordingly to company protocols.
- Educate Clients and their families concerning potential side effects & adverse effects of medications.
- Communicate with Clients and their families regarding any medication concerns.
- Notify physician of the occurrence of side effects, adverse effects, and/or allergic reactions.
- Perform in depth Nursing Assessments.

Support the Community Nurse Manager with Welness Center needs, including:
- Assessing Residents health related concerns & triaging needs.
- Performing basic first aid, minor wound care, and assessment of vital signs.
- Resident education re: medication and/or nutrition questions.
- Responding to Resident falls and/or 9-1-1 calls.
- Weekly Lab draws, including completing Lab Requisition forms and reporting results to the appropriate physician.
- Support our Nursing Compliance related needs. From correction documenting, updating, auditing, and uploading any necessary documentation on a weekly & monthly basis.
Qualifications
- Must have experience with caring for the geriatric population, especially individuals with dementia diagnoses and/or receiving hospice.
- Must enjoy working in a fast paced environment. Be skilled at critical thinking & problem solving. Be detail oriented. Have great written & verbal communication skills.
- Must be a LPN, RN, or BSN.
- Must have the flexibility to work ON-CALL on the weekends, 2x/month.
- Must be willing to commute to The Village of Victory Lakes in Lindenhurst, IL.
