Job Summary
We are looking for someone to join our team as a Qualified Professional. The Qualified Professional will assist in verifying eligibility for people who are homeless or at risk of homelessness for funding and services to afford, find, and keep housing. A Qualified Professional is responsible for signing the Professional Statement of Need (PSN).
The Qualified Professional will complete eligibility requirements and PSNs as needed at a per unit rate, depending on availability. This contract role is considered very part time, and can range from 1 PSN per week, up to 10 PSN. The PSN averages 30-45 minutes to complete the eligibility and required documentation.
** The Qualified Professional role is considered fully Remote, however work must be completed within the state of MN**
Responsibilities.
- Conduct health assessments including homeless or at risk of homelessness assessment
- Sign and complete PSN and individual must meet eligibility requirements on the PSN
- Work collaboratively with the Housing Stabilization Services (HSS) department, and other service providers to provide person-centered care
- Maintain accurate and complete service records, including medical history, and progress notes
- Ensure compliance with legal and ethical standards for documentation
- Adhere to state and federal laws, regulations, and professional standards
- Maintain licensure and certifications through ongoing education and professional development
Qualifications
- Licensed requirements: Must meet one of the following and possess active license:
- Licensed Mental health professional,
- Licensed school psychologist,
- Licensed physician,
- Licensed physician's assistant,
- Licensed nurse practitioner,
- Licensed psychologist,
- Licensed independent clinical social worker,
- Licensed professional clinical counselor,
- Licensed psychologist, licensed marriage and family therapist, or
- Licensed psychiatrist
- Strong clinical skills and the ability to work independently
- Excellent communication and interpersonal skills
- Commitment to patient-centered care and continuous learning
