What are the responsibilities and job description for the In Home and Community DSP position at Opportunity Partners?
As an In Home and Community DSP you will have a case load of 6-10 individuals, with disabilities, that you consistently meet with on a weekly basis. You will be assisting in daily living and community access so that each person meets their personal goals and is able to achieve their best quality of life.

Responsibilities:
- Promoting independence through individual skill building
- Medical coordination, transportation and documentation
- Complete all daily, weekly, monthly and annual documentation
- Assist with community integration and engagement
- Communication and coordination with team members
- Advocate with and for individuals for independence
- Help individuals identify and reach personal outcomes
- Recreation – Get paid to do fun things!!
Requirements:
- High school diploma or equivalent
- Must be 18 years or older
- Must have a valid driver’s license, motor vehicle and insurance
