MDS Nurse
Fulltime: Mon - Fri 9a - 5:30p
- Qualifications:
- Must be a Registered Nurse or Licensed Practical Nurse
- MDS experience is preferred
- At a minimum must have knowledge of current state and federal regulations, particularly the following sections of Appendix PP:
- Resident Assessments
483.21 Comprehensive Resident Centered Care Plans

- Physical Requirements:
A. Work in well-lighted/ventilated office areas and throughout the nursing service areas.
b. Sitting, standing, bending, lifting, and moving intermittently during working hours.
c. Must possess sight/hearing senses or use prosthetics that enable these senses to function adequately so that the requirements of the position can be fully met.

- Responsibilities:
- Coordinate the RAI process, which include, at a minimum, the following elements:
- Maintaining the Minimum Data Set (MDS) OBRA and PPS assessment schedule
- Completion of MDS assessments
- Care Area Assessment process
- Care Plan development
- Care Plan implementation
- Evaluation of residents for change in condition: Significant Change in Status or Interim Payment Assessments
- Conducts quality control checks of MDS assessments for completeness, accuracy, and timely completion.
- Coordinate with Navihealth Skilled Inpatient Care Coordinator (SICC) for renegotiations of authorized case mixed groups (CMGs) as needed.
- Maintain confidentiality of sensitive information.
- Possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, that are necessary for ensuring the accurate and timely completion of the RAI documents.
- Be knowledgeable of regulations, practice standards, procedures, and laws, regulations, and guidelines pertaining to the RAI process.
- Complete and maintain the Level of Care Determination through CHAMPS to ensure that admitted residents are eligible for nursing home stay.
- Maintain the Resident Matrix on the electronic health record in the absence of the MDS Coordinator.
- Perform any additional duties assigned by the Administrator, Director of Nursing, and MDS Director.
