POSITION SUMMARY: The Information and Referral Specialist (I&RS) will be a part of the Marketing and Business Development (M&BD) team, working with the M&BD Director. The I&RS will work with the team to provide support and information to residents throughout Maryland. In addition, they will provide assessments and intake visits for the Case Management Division. The I&RS will be expected to travel as needed throughout their region for meetings and events, and to other regions when additional coverage is required.

QUALIFICATIONS

  • Bachelor’s degree in the Human Services field.
  • One year experience in human services.
  • Proficient in the use of Microsoft Office or an equivalent computer program.
  • Demonstrate the ability to maintain confidentiality.
  • Ability to communicate effectively.
  • Must be able to work independently, prioritize work activities, and use time efficiently.
  • Active CPR/First Aid certification
  • Active driver’s license, auto insurance, reliable transportation, and the ability to travel to patients' homes within the assigned geographic area
  • Clear criminal background check
  • Negative Hepatitis B test
  • Negative tuberculosis test within the last 12 months or clear chest x-ray within the last five (5) years
  • Computer proficiency
  • Three professional or personal references
  • Not been determined to be:○ On the Medicaid exclusion list○ Convicted of a crime in violation of Criminal Law Article 14-101 annotated code of MD.○ In violation of Health-General Article 7-1102 annotated code of MD○ Pass a criminal background check.

DUTIES

  • Performs work that is predominantly intellectual and requires the use of discretion and judgment regularly.
  • Performs comprehensive assessments for Developmental Disabilities Administration (DDA) case management.
  • Meets with individuals and families for all initial case management intake.
  • Represent Optimal Health Care (OHC) at community-based events, including job fairs, Transitioning Youth (TY) fairs, health fairs, and other community resource events.
  • Attends training to develop extensive knowledge of communities, resources, and school services.
  • Provides support and partnership for other agencies in their region, establishing collaborative relationships that enhance community services and spotlight OHC’s commitment to communities in Maryland.
  • Meets with families and individuals, as well as provides phone consultations for individuals looking for information about services within their community.
  • Strives to become proficient in the requirements of other state and Medicaid programs.
  • Visits local schools, Head Start programs, pediatrician offices, hospital/NICU centers, and daycare centers to provide information on all of OHC services and offer I&R services for families and providers as well as offer any other resources that may be of benefit to those we support.
  • Maintains client information, notes, and documents in the database used by this agency.
  • Maintains proper use of company equipment including a laptop and mobile phone.

WORK CONDITIONS

  • May be exposed to unsafe neighborhoods and/or unhygienic environments.
  • As an "essential" employee, you may be called upon to assist with natural disasters and/or public health emergencies.

Pay: From $26.00 per hour
