Job Title: Registered Nurse
Job Summary: Responsible for working with team members to help treat patients with various injuries, illnesses, or disabilities. To assess and identify patient needs, implement the same and monitor their medical plan and treatment. To also ensure that the care planned is conducted according to the Company policies and procedures. Responsibilities & Duties:
- Formulate a plan of care by conducting an initial, comprehensive assessment of the patient to determine their home care needs. Provide a complete physical assessment and history of current and previous illness.
- Coordinate total plan of care with other physicians involved in care and help to achieve and maintain continuity of patient care by planning and exchanging information with physician, patient, family, and other health care professionals involved.
- Administer medications and treatments as prescribed by the physician.
- Assist the patient with activities of daily living that enables them towards self-sufficiency.
- Maintain a high level of integrity of patient confidentiality. Follow safety and health policies & procedures.
- Achieve and maintain continuity of patient care by communicating information, including changes in the plan of care, new orders, lab results, and new referrals as required.
- Complete and record all relevant care information accurately in a timely manner.
- Counsel and educate patients and their families on treatment plans and meeting nursing needs
- Educate patient and their family members to maintain a safe environment for the patient.
- Work in cooperation with interdisciplinary groups to meet the emotional needs of the patient and their family.
- Participate in on call duties as defined by the agency.
- Ensure and coordinate for all equipment and other medical supplies are available
- Use critical thinking skills to complete patient care needs independently or by appropriately delegating.
- Other duties as assigned.
Required Qualifications:
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting
- Current licensure in state and CPR certification
- Bachelor’s degree, with one (1) year of home health care experience preferred
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements
Experience: Minimum one year in Home Health
