Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community. Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.
Because many of our patients are frail and elderly, we deliver care primarily in the comfort of their home. Our Program is also offered to eligible patients at no incremental financial cost to them. We are not a fee-for-service practice; we benefit economically only if we deliver high-quality patient outcomes and satisfaction. At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

The Community Health Worker is a pivotal member of the Landmark Interdisciplinary Team. This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients’ and caregivers’ trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.
This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:30am - 5pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at 5015 Campuswood Dr suite 107, East Syracuse, NY 13057 .
Primary Responsibilities:

• Conduct home visits and/or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:
• Resource referral and connection
• Quality measure documentation, support, and follow up
• Telemedicine facilitation and set up
• Collect information regarding patient conditions using standardized checklists and templates
• Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction
• Actively participate in weekly interdisciplinary team meetings

Required Qualifications:

• High School Diploma/ GED (or Higher)
• Reside in Binghamton NY area
• Travel up to 25%
• Required to have a dedicated work area established that is separated from other living areas and provides information privacy
• Ability to keep all company sensitive documents secure (if applicable)
• Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
• Knowledge of basic Medical Terminology
• 3 years of experience in any of the followings job categories: Social Worker; Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician. Candidates with experience similar to those listed here are encouraged to apply as well
• Knowledge and proficiency of basic Word, Outlook and Excel applications
• Access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation.

Preferred Qualification:

• Geriatric or primary care experience, or experience in institutional settings of care such as long-term care/skilled nursing facilities

Soft Skills:

• Exceptional interpersonal skills. Superb written and oral communication skills, and comfort level with technology
• Ability to conduct independent home visits
• Comfort with conducting independent home visits
