What are the responsibilities and job description for the Registered Nurse position at OpusCare?

We are dedicated to providing patients and their families excellence in palliative care and support services when faced with a terminal illness. We provide a customized individual program where “Dignity Care” is our number one priority. OpusCare strives to meet the patient’s physical, emotional, and spiritual needs and provides support for the entire family during the illness and for at least one year after a patient’s death. Most hospice care is provided at home, where people most want to be when they are ill. In addition, hospice care can also be provided in a nursing or residential facility and in certain circumstances, in a hospital or hospice management residence.

At the highest level of, OpusCare Employees must be committed to hospice philosophy and mission and provide outstanding customer service to both internal and external customers. As a member of the Interdisciplinary team, the “Field Nurse” works under the direction of the “Director of Nursing” and/or in her/his absence the “Assistant Director of Nursing Supervisor”, and the patient’s/Hospice physician in the provision of skilled nursing care & services, assessment, planning and care in order to maximize the comfort and health of patients and families. Responsible for providing nursing care and services, assessing and identifying patient/family needs, and providing skilled care and supportive services in accordance with the patient’s physician’s orders and plan of care and Opuscare policies and procedures.

-Assume nursing responsibility for a patient/family caseload that includes the assessment, planning, implementing and evaluating phases of the nursing process.

-Initiate, and relate to the office staff, communication with attending physicians, other Hospice staff members, and other agencies, as needed, to coordinate optimal care and use of resources for the patient/family.

-Maintain regular communication with the “Team Director” to review caseload.

-Maintain regular communication with “Field Services Liaison(s)” staff and or the Field Nurse Supervisor/On-Call Supervisor to review the daily caseload and patient’s disease progression/reports, as necessary.

-Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

-Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.

-Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.

-Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, or by providing direct care, as appropriate.

-Meet regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support and exchange feedback aimed toward enhancing professional growth.

-Attend interdisciplinary team conferences and patient conferences to facilitate discussion of issues from caseloads for full staff discussion, consultation, and evaluation.

-Provides information to department director(s) of unusual or potentially problematic patient/family issues, assisting during the problem resolution phase and providing appropriate follow-up intervention.

-Assumes the responsibility of sharing the on-call coverage to patient/families on rotation, or as requested.

-Provides appropriate care and support at time of death, ensuring proper procedures are followed.

-Participates in OpusCare orientation and training programs for professional staff.

-Participates in agency and community programs, as requested, to promote professional growth and understanding of Hospice care.

-Demonstrates knowledge of Hospice rules and regulations and of program policies and procedures.

Job Types: Full-time, Per diem

Pay: $40.00 per hour

Experience:

  • Registered Nurse: 1 year (Required)
  • Acute setting: 1 year (Required)

License/Certification:

  • BLS Certification (Required)
  • RN (Required)

  • Decatur, GA 30035: Relocate before starting work (Required)
