Certified Nursing Assistant

Job Specific Responsibilities
- The Certified Nursing Assistant (CNA) directly reports to the shift Team Leader.
- Delivers medical/non-medical care to patients as directed by the team leader and the Plan of Care.
- Always maintains patient comfort and safety to include proper positioning and turning of hospice residents (every 2 hours or more frequently as indicated)
- Follows all departmental policies and procedures as well as safety, environmental, and infection control standards.
- Record all entries on flow sheets, notes, charts, etc., prior to end of shift.
- Report all changes in a resident’s condition to the Team Leader as soon as possible.
- Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
- Report injuries of an unknown source, including skin tears.
- Report and document all accidents and incidents to Team Leader. Ensure incident reports are signed by employee submitting report and team leader prior to end of shift.
- Abide by HIPPA regulations
- Assist with transferring residents
- Participate in and receive the handoff report upon reporting for duty.
- Assist residents with ADLs including but not limited to:
- Daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
- Bath functions (i.e., bed bath, tub or shower bath, etc.) as directed
- Dressing/undressing as necessary
- Hair care functions (i.e., combing, brushing, shampooing, etc.).
- Nail care (i.e., clipping, trimming, and cleaning the finger/toenails). Note: Does not include diabetic residents
- Shave male residents.
- Assist resident with bowel and bladder functions (i.e., take to bathroom, urinal, portable commode, etc.).
- Provide eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aids, etc.) as instructed.
- Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
- Perform after meal care (i.e., clean resident’s hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.).
- Provide feeding assistance as indicated (i.e., cutting foods, consuming, assist in dining room supervision, etc.).
- Keep residents dry (i.e.: change and launder clothing, linen, etc., when it becomes wet or soiled).
- Make beds and change bed linens when wet or soiled. Keep linens tight to avoid wrinkles from forming under the resident.
- Weigh resident and obtain vitals per Plan of Care.
- Answer/Clear resident calls within 5 minutes.
- Check on all residents throughout shift as indicated by Plan of Care (every 2-4 hours)
- Assist with the care of the dying resident. Provide post-mortem care as instructed
- Ensure residents have access to fluids in clean containers filled with fresh water or juice (on each shift), and within easy reach of the resident.
- Serve between meal and bedtime snacks as indicated
- For insulin dependent diabetics, report concerns about food items to Team Leader.
- Perform all assigned tasks in accordance with established policies and procedures
- Report all complaints and grievances made by residents to Team Leader
- Participate in all mandatory training and compliance programs.
- Maintain current certifications (CNA and CPR)
- Report all unsafe/hazardous conditions, defective equipment, safety concerns etc., to the team leader immediately.
- Follow universal precautions, donning and disposal of proper PPE
- Immediately notify the team leader of any resident leaving/missing from the Community.
- Must be able to lift a minimum of 50 pounds.
- Must assist in the evacuation of residents during emergency situations
